E-DOCUMENTS

From October 1st the list of forms/applications that can be submitted in the form of E-Document is updated and expanded with additional applications (My.PG->Documents->eDocuments->Add). If the required application is an e-Document, it should be submitted only in this form (paper applications/scans will not be accepted).

List of available E-Documents:

Application for an Individual Study Plan or Program- the rules for granting individual organization of studies can also be found in the faculty rules: https://zie.pg.edu.pl/en/students/regulations

• Bachelor's, engineer's/master's thesis topic assignment form,

• Application to change the supervisor/supervisor and the topic of the diploma thesis,

Application for extension of the exam session - you can submit it in the event of an excused absence from an exam or a credit test,

Application for an extension of the deadline for submitting the diploma thesis,

- Request for change of foreign language,

- Application to take an examination in a language other than the language being studied,

Leave application - sick leave - if you want to apply for sick leave, attach a certificate or sick leave,

Leave application - other (dean's leave, random leave, break before repeating, etc.) –

Application to the Rector for long-term leave (over 1 year),

Application for registration for the next semester - if you have a debt exceeding 12 ECTS points,

Application for registration after DEAN'S leave (dean's leave, accidental leave, break before repeating, etc.),

Application for registration after SICKNESS leave - if you want to report your return from leave (in the case of sickness leave with an attached doctor's certificate),

Installment application for the winter semester- if you want to pay in installments,

- Application to the Rector for changing the basis of study - for foreigners studying for a fee,

- Application for a fee refund,

Application to the Rector for the remission of fees for foreigners,

Application for transfer to a different field of study (within the Faculty) - read the rules:

Application for transfer to another Faculty and field of study ( Approval from the Dean of the transferring faculty and a study process card signed by the Dean's office must be attached.)

General application to the Dean - if you want to apply another matter not included in the E-Documents,

General application to the Rector, ​​​​​​

Application for issuing a copy/additional copy of the diploma of completion of studies in English, and/or a copy/additional copy of the completion supplement in English,

The Dean's final decision will be visible in the application with the "completed" status.

You will be informed by e-mail or text message about the possibility of collecting the ready Certificate (if you have agreed to receive text messages). Sometimes, the waiting time for a ready application may be long.

• To collect the Certificate in person, you must make an appointment at the Dean's Office via website https://polgda.nakiedy.pl/site/?c%5b%5d=4427

• We also send certificates to the e-mail address as a scan (please add a comment while applying)

KPA DOCUMENTS

We would like to inform you that in accordance with the order of the Rector of GUT No. 39/2023 of September 1, 2023, from October 1, 2023, in administrative proceedings regarding removal from the list of students, we only use the university's Moja PG system - KPA Documents module - to submit and deliver letters.

Resignation from studies should be submitted in the form of an E-Declaration of Resignation from Studies (Moja.PG->Documents->eDocuments->Add).

IMPORTANT!

• If an eNotification of initiation of deletion proceedings (eZawiadomienie o wszczęciu postępowania w sprawie skreślenia) appears on your account on the moja.pg portal, you can submit an e-application regarding the deletion (ePodanie w sprawie procesu skreślania) process under the KPA procedure within 7 days from the date of receipt.

• If an eDecision on expulsion (eDecyzja) appears on your account on the moja.pg portal (does not apply to resignation based on the student's declaration) - you can submit an eApplication for reconsideration of the case (eWniosek o ponowne rozpatrzenie sprawy) under the KPA procedure (Student-Documents-Submit an application) within 14 days from the date reception.
